Terms:

    Payment must be made by one of the following methods.
    1. Credit Card: American Express®, Discover®, MasterCard® or Visa®
    2. Cashier’s Check, Money Order or personal check sent before shipment.
    3. Invoice billing will be considered following completion of credit application

Shipping Policy:
  • Orders will be shipped via UPS or common U.S. carrier.
  • Special shipping instructions, as supplied by the customer, will be followed and charged accordingly.



Shipment Time Frame:

  • Orders placed by 12 noon CST will normally ship same day, pending credit approval and inventory status.
  • Products are typically shipped 2 day ground.



Warranty Information:

    Limited Warranty: Evenup, LLC  warrants all products to be free of manufacturing defects. Each product is intended for use by a single patient.

Returned Goods Policy:
    1) Please report and discrepancies or damage claims within 10 days of receipt of product.

    2) Returns will be accepted up to 10 days from date of shipment.  Any item that has been used may not be returned unless defective.

    3) Prior to shipping the merchandise back to Evenup, LLC contact Evenup, LLC  for a returned goods authorization number. This authorization number must appear on all paperwork accompanying the shipment.

    4) All returns, except for defective merchandise or errors in shipping by Evenup, LLC  must be returned to Evenup LLC, freight prepaid.

    5) Credit will be issued to your account upon receipt, inspection and acceptance of the product at Evenup, LLC . Product that has been damaged or cannot be restocked will not be accepted.
